What information is collected

Clinic participation requires a client to provide relevant original documentation. Pending the client’s needs, this may include their birth certificate, passport, driver’s license, Medicare card, SIN card, and/or student identification. This documentation is the property of the client and is not kept (physically or digitally) by the Clinic following an appointment.

Where information is collected from
The Clinic operates under a virtual model. Therefore, copies of a client’s original documentation may be collected by Clinic staff over email.

Why information is collected
​
Documentation is collected as part of the client’s transitioning process. A client’s application to change their legal name or gender marker designation requires information found in their original documentation.

Who information is shared with
​
PBSC UNB is committed to protecting the individual privacy of each client participating in the Clinic. A client’s information will only be collected by Clinic staff and shared between relevant Clinic staff. None of a client’s information will be kept or shared externally except with the client's express consent.
